Preventing Shoulder Injuries - A Physical Therapists Perspective

It allows us to manipulate objects in an infinite number of directions at various speeds. The shoulder is the most mobile joint in the body and provides many degrees of motion. This freedom of motion allows us to reach high and low, far and near for objects in space. The large amount of shoulder mobility does not come without a price. Increase in shoulder or (Glenohumeral Joint) mobility leads to a decrease in the stability of the joint. When a joint is unstable, it is more likely to suffer an injury. The most common injury to the shoulder is from a repetitive movement on an unstable joint. This movement can vary from typing at a computer with poor posture to an unstable tennis or golf swing. Shoulder injuries are not discriminatory to a specific sport or action. They affect both male and females athletes and non-athletes alike. The following guidelines in this article will help to prevent the possibility of developing a shoulder injury due to overhead sports, poor posture and improper weight training. Anatomy The shoulder complex is made up of four joints. The glenohumeral joint, acromioclavicular joint, sternoclavicular joint and the scapulothoracic joint. Aside from these joints, there are other anatomical structures that play a large role in normal shoulder function. These include the cervical spine (neck), thoracic spine (upper back), clavicle and ribs. If any one of these structures is not functioning properly, they will have a direct negative impact on the shoulder complex. Full shoulder flexion in the saggital plane in a normal adult is 180. This movement is not accomplished by the glenohumeral joint alone. In fact, full arm elevation involves thoracic extension, the 1st & 2nd ribs must depress and move posterior, the scapula (shoulder blade) must rotate upwards, the clavicle needs to elevate and roll back and finally the glenohumeral joint needs to glide downward and roll upward. Mechanism of InjuryIf any one of these movements does not occur, the shoulder joint is susceptible to injury. The most common area of injury to the shoulder occurs in what is known as the sub-acromial space. This space is located just under the acromioclavicular joint. There are many structures passing through this space leading to a very tight passage. Included in this space is rotator cuff tendons, neurovascular bundles and other connective tissues. If any one of these structures is irritated or injured, inflammation occurs. Inflammation will take up valuable real estate in the sub-acromial space leading to possible impingement of the tendons, nerves or vasculature. The Rotator Cuff Muscles play a vital role in shoulder stability and injury prevention. The job of the four rotator cuff muscles (infraspinatus, supraspinatus, teres minor & subscapularis) is to synergistically contract at the same time to hold the shoulder centrally in the glenoid fossa to maintain an precise axis of rotation. If any one of these muscle is weak or injured, the shoulder will migrate out of position and either elevate or move forward in the joint capsule. This movement reduces space in the sub-acromial region which often leads to rotator cuff impingement. The rotator cuff tendons rub against the acromion causing friction, inflammation and eventually will tear if not resolved. The three rules of prevention. 1. Proper posture 2. Good body mechanics 3. Well rounded exercise program We are a product of our environment. A normal day for someone consists of the following: Wake up and head down to the breakfast table. We may sit for 20 minutes eating breakfast. Shower, get in the car or train and sit for a daily commute to work. Many people are sitting at a desk working on a computer for at least 5 hours daily. Then we get back in the car for the commute home. Dinner time arrives and again we sit down at the table for an hour. Following dinner maybe we sit and watch television or sit reading a book. Do you see a common theme here? Most of us are sitting all day long.When positioned in a seated position, gravity works against us to pull our head and shoulders forward. This posture leads to a decrease in sub-acromial space and places us at risk for shoulder impingement. How can this be avoided? Having a good ergonomic chair that places the hips slightly above the knees will orient the back in a good position. Arm rests are important for taking pressure of the shoulder girdle. The neck should be positioned in neutral by tucking the chin back in an attempt to lengthen the neck. The shoulders should be positioned down and back. Try to imagine bringing the shoulder blades toward the opposite back pocket. A stretch should be felt across the chest and shoulders. Body Mechanics Lifting, throwing and exercising using proper positioning can make a big difference in preventing injury. When lifting something overhead, the shoulders must stay down. If the shoulder moves upward towards the ears, you are increasing risk for impingement by closing down the sub-acromial space. Repetitive movements using improper body mechanics leads to shoulder injury. If an athletic movement such as the golf swing or tennis serve leads to pain in the shoulder, dont try to work through the pain. Seek professional advice from a physical therapist or a sports doctor. You may avoid a serious injury by inquiring when the discomfort first begins. Waiting until you can’t bear the pain usually means the injury is serious and may require surgery. Shoulder Exercises Exercising the shoulder to prevent injury should be conducted three times per week. There are many variations of exercises that can be used to strengthen the rotator cuff muscles and back musculature. Seated rows and lat pull-downs are great ways to strengthen the muscles around the shoulder blades. Keeping the shoulders down and pinching the blades together is the proper mechanics during this movement. Shoulder external and internal rotation will keep the infraspinatus, teres minor and subscapularis strong. Shoulder elevation to the front and side will strengthen the deltoids & supraspinatus. Make sure the shoulders do not hike up toward the ears with this exercise. Finally, proper stretching to keep the shoulders and pectoral muscles loose is vital to healthy shoulders. A door or corner stretch is great to open up the thoracic spine. This is accomplished by placing both forearms along the frame of the door with the elbows at shoulder height and keeping the abdominals tight. Gently bring the chest forward into the door until a comfortable stretch is felt across the chest and shoulders. Do not bounce or arch low back. This stretch should be held for a minimum of 30 seconds. In summary, proper posture and body mechanics is vital for preventing shoulder injuries. A well balanced exercise program that promotes flexibility, rotator cuff and back strengthening and precise body mechanics during exercise will allow persons to excel in their respective activities minimizing the risk for shoulder injury. What Should I Do If I Am Arrested?

By Philipp Kostins While no one ever plans on being arrested, it is a good idea to know what you should and shouldn’t do in case it ever happens. By understanding what is expected of you and what circumstance can make your situation worse, you will be better able to handle the distress and fear that comes with being arrested. First of all, there are certain things that you should do if you ever find yourself in this scary situation. Do attempt to stay calm and try to only speak to the police officer in a respectful, polite manner and tone of voice. By staying calm you will be better able to assess the situation for what it really is and better able to recall all of the details of your arrest later for your attorney. By being polite and respectful you will cut down on the chances that the arresting officers will become angry or aggressive, which could lead to your being injured or harmed. Do give the basic information that the officers are asking for without being surly or rude. This information includes your name, address, and telephone number, the name of an immediate family member and their phone number and the name of the place of your employment. Not only do the police officers need this information to file the forms of your arrest, they will also need it when setting your bail. Do exercise your right to remain silent. You are not required by law to answer any questions that could incriminate you and you do not have to speak to the police, district attorney or anyone unless you are in the presence of your lawyer. When You Must Dine on “Fast Food”

By Michael Callen I am one of the fortunate few who get to work from a home-office most days of the year. Because of the nature of my work, my physical location is not important to the task at hand. As such, I also have the opportunity to better control my food choices and eating habits during the work day. If you are in this category, be thankful that you are because — for most people — it is much more difficult to eat well during the work day. Sometimes, however, my work requires my physical presence in the office — often for many days at a time. This week was such a week. Yesterday, as lunch time came and went, the pangs of hunger began to gnaw at me and I finally decided to run over to the local McDonalds for some lunch. Since I was very hungry, I reminded myself to order thoughtfully. When those of us fighting the Battle of the Bulge are visiting a fast food restaurant, there are some really obvious things that we must do to avoid overeating and/or choosing the wrong kinds of food. These “obvious” things include to not order anything that is “deep fat fried” and to not “super-size” any order. It struck me, though, as I sat there “inhaling” my cheeseburger, that there are many other things that we can do that will also help prevent over-consumption of the wrong kinds of foods. The first thing to consider is what you drink. Most fast food companies will provide “ice water” at no charge when you are making a food purchase. Not only can you save some money, but you can drink more water (please see the www.weeklyweightlosstips.com archives to read more on drinking water), which is something that you already know to do. Many wise fast food restaurants are also selling your favorite brands of bottled water, which can typically be refilled at the beverage kiosk if you are still thirsty when you finish your meal. Not only is this important when comparing beverage choices (e.g., soda vs. water), but also when you consider the fact that fast food contains much more salt than what we typically would use on our own food at home. If you have been reading these articles for some time now, you have likely read some compelling reasons why we need to limit our sodium intake. Sodium causes us to retain water (which weighs about seven pounds per gallon) and the only way to safely regulate this (sodium and water retention) is to drink more and more water. The next thing to consider is the condiments. Many fast food restaurants are suggesting salads as a low-calorie option to their traditional offerings. These are a great way to avoid higher fat, lower fiber alternatives, but with salads the dressing must be scrutinized, or else you may end up not saving calories at all. Dieticians suggest choosing the dressing type wisely, as well they recommend using the smallest amount possible. If you order items other than salads, the same thing is true. Mayonnaise if a very high-fat food that should be avoided. Most restaurants will allow you to “special order” a small amount of your condiments (“light on the mayo, please…”) or just those that are lower in fat and calories. Mustard is a low fat, low calorie condiment, though it may not be suited to everyone’s individual taste. The last thing that I would recommend that you keep in mind when visiting a “fast food” restaurant is to not get wrapped up in the “speediness” of the environment. One of the worst dining habits is to eat your meals quickly. The very nature of the fast food shop makes it difficult to slow down and enjoy your meal. Make a conscious effort to sit down, relax, and savor every bite. Don’t allow yourself to get caught up in the hustle and bustle of the surroundings. When you sit down at the table with your food, tell yourself that you are going to eat slowly, savoring your food, or you are likely to eat too quickly and too much. You may not always be able to avoid eating at a fast food restaurant, so when you must do so, prepare yourself before you get in line to order. Don’t let your hunger cause you to over-order, make wise food and beverage choices, and when you eat, take time to savor your food. Lead Generation System Secrets of a Lead Generation Coach - Part 2

By Fraser Hay A Lead Generation System can be split in 5 key areas 1. Set your objectives 2. Allocate your time and budget 3. Select your marketing approach 4. Test 5. Evaluate 1. Setting Objectives. The first part of creating your Lead Generation System is to set your objectives. One of the best ways to decide upon what your objectives are going to be, is to do a marketing audit, to identify what has already worked, what hasn’t worked, what your marketing achievements have been, identify your disappointments, what you’ve learned, and what you are going to do differently over the next 12 months. 2. Allocating your time and budget. The next part of your lead generation system is to decide on a budget and schedule that you dedicate to your marketing. Many people tend to view marketing as an expense instead of an investment, and most businesses know how many sales they need, but very few actually know how many leads they need. Ironically, this is what they’re marketing is spent on - generating leads. There is a very useful formula for working this out - a) What income do you require per week b) What is the profit per sale/transaction you offer c) Divide a) by b) to give you the number of sales you need d) Decide on what your closing ratio has been and is (e.g. 1:2, 1:5, 1:10) e) Multiply c) by d) to determine the number of leads you need f) You will know how much available marketing spend you have, so simply divide that by e) to determine how much per lead you can sensibly afford to spend. 3. Selecting a marketing approach Your Lead Generation System can utilise a number of methods for generating leads. So the question is. Which approach do you use first? The answer is revealed in part 1 of the process - do what has worked previously for you. If however cashflow is good, and you have calculated your available budget and schedule using the very simple formula outlined in part 2. You may wish to test one or two new approaches on a limited basis, before deciding on which method to “roll out”. 4. Testing Many people have a lead generation system that utilises just one marketing approachwhat happens if that channel, tactic, or strategy fails for whatever reason? They have to try a new approach. (Even “Networking” as a channel has lots and lots of indirect and hidden marketing costs that people tend to forget about - train fares, tube fares, taxis, coffees (Lates, mocha and espressos which ever one you choose) , lunches, Beer, chardonnay…it all adds up and drains your cashflow. ) How cost effective (and successful) is your current marketing strategy? There are 3 fundamental errors people tend to make when testing a new marketing approach. They focus on what they OFFER the client. They focus on what they DO for the client They focus on THEMSELVES - not the client. Whichever Marketing approach you test, ensure you rigidly stick to this simple formula - a) Identify your target market and audience - and build a prospective client “profile” b) Ensure you have a list of client problems that you know you can address c) Ensure you know your product or solution that you are offering your clients d) Have a clearly defined pricing strategy to help your casfhlow whilst the bigger deals take time to “land” and “close” e) Have a well articulated message for your target audience that highlights b) and c) above. f) Ensure you have implemented 2) in your Lead Generation System before you begin testing. g) Decide on a marketing tactic - and try it, ideally for a period of 4, 8 or 12 weeks. 5. Evaluating your Lead Generation System Remember part 1 - your objectives? Use these as your Key Performance Indicators or mile stones to monitor the effectiveness of your lead generation system and marketing campaign. Here are some indicators you could use to monitor the effectiveness of the system - No, of new ezine subscribers No. of new signups or downloads for your FREE “teaser” offer No, of words or square centimetre columns (scc) coverage you got in the press No of Appointments No. of business cards collected No. of referrals No. of affiliate signups No. of qualified sales leads No. of sales Determine which of the marketing approaches you are using is generating the most progress and business for you. If you are continually using a strategy that Is ineffective - drop it, and test a new approach. Remember, in a condition of need, when you’re short of new business, it is hard to hide that, and when you’re “needy” that feeling of neediness, even desperation is apparent, because you are preoccupied with you, and your focus is on you and you tend to talk about what you HAVE, what you can DO, not what the client GETS or NEEDS. Your prospective clients are preoccupied with themselves, and their problems too, that’s why it’s important your lead generation system is set up correctly so that it isn’t YOU focused and it is client focused. Setting up a Lead Generation System can and does take time, but once created, can run like a well oiled engine, if the process above is followed. Many self employed service providers who don’t have a lead generation system in place for their business very quickly believe that marketing is a waste of time and even begin to doubt whether self employment is actually for them. A Lead Generation System can and does work, but you have to start thinking differently and then taking consistent action with your new thinking. This new approach can be place you out of your comfort zone, can be perceived as HARD WORK, even frustrating on occasion - but it can pay enormous dividends, and even increase the Life time value of a client by as much as 100 - 500%.
